This Service Agreement outlines the terms and expectations for participation in the Provider Ready™ service. By enrolling in Provider Ready™ and checking the acknowledgment box on the intake form, you agree to the terms below.

1. SCOPE OF SERVICES

Provider Ready™ is a structured career transition service designed to support healthcare providers as they move from resume preparation through onboarding readiness.

Services may include, as applicable:

• Resume Refresh

• Credentialing Ready™ components (credentialing-style CV, CAQH worksheet, timeline tracker)

• Interview preparation guidance

• Onboarding and start-date readiness guidance

Provider Ready™ is time-bound and scope-limited. The exact deliverables are based on the service description in effect at the time of enrollment.

2. WHAT THIS SERVICE IS NOT

Provider Ready™ does not include:

• Medical advice or clinical decision-making

• Supervision, precepting, or clinical oversight

• Legal advice, credentialing approval, or employment placement

• Guarantee of job offers, credentialing approval, or start dates

• Unlimited access or on-demand support

All services are advisory and educational in nature.
3. CLIENT RESPONSIBILITIES

To ensure timely and effective delivery, the Client agrees to:

• Provide accurate, complete, and truthful information

• Complete intake forms and document submissions in a timely manner

• Respond to requests for clarification or revisions within reasonable timeframe

Delays caused by incomplete or late Client responses may impact timelines and do not constitute a breach of this agreement by the Company.

4. TERM OF ENGAGEMENT

Provider Ready™ is delivered over a defined engagement period (typically 4–6 weeks, depending on scope and timing).

The engagement concludes once agreed-upon deliverables have been provided or the service period has ended, whichever occurs first.

5. COMMUNICATION & ACCESS

• Primary communication occurs via email or scheduled calls, as outlined during onboarding

• Response time is typically within 24–48 business hours

• This service does not include emergency support, text messaging, or real-time availability

6. FEES & PAYMENT

• Full payment is required before work begins

• Fees are disclosed prior to enrollment

• Once work has commenced, fees are non-refundable

No refunds are issued for completed work, partial completion, or unused time within the engagement period.

7. INTELLECTUAL PROPERTY

All templates, tools, and materials provided by the Company remain the intellectual property of Letitia R. Stewart Consulting, LLC.

Clients may use delivered materials for personal professional use but may not reproduce, distribute, or resell them.

8. LIMITATION OF LIABILITY

Provider Ready™ provides professional career support only. Outcomes related to employment, credentialing, onboarding, or workplace performance depend on multiple external factors beyond the Company’s control.

The Company is not liable for decisions made by employers, credentialing bodies, or third parties.

9. TERMINATION

The Company reserves the right to terminate services if a Client:

• Violates the terms of this agreement

• Engages in abusive or inappropriate behavior

• Fails to meet reasonable participation requirements

In such cases, completed work will be delivered, and remaining fees are not refundable.

10. GOVERNING LAW

This agreement is governed by the laws of the state in which Letitia R. Stewart Consulting, LLC is registered.

11. AGREEMENT ACKNOWLEDGMENT

By enrolling in Provider Ready™ and submitting the intake form, the Client acknowledges that they have read, understood, and agree to the terms of this Service Agreement.
